Appointment Instructions due to COVID-19

Yes, we are open for essential eye care visits. If you have an appointment, and you wish to reschedule to a later date, please call us at 262-421-4412 and we will be happy to accommodate you.

If your eye concerns are urgent, but are uncertain about coming, please do not hesitate to request a telephone consult.

If you answer YES to ANY of the following, you will need to reschedule

  • Do you have a fever or cough or other respiratory symptoms?

  • Have you had contact with someone confirmed to have COVID-19?

  • Have you traveled to a high-risk area for COVID-19?

Patient Only or Patient + 1

  • Come by yourself, or at most, with one other person

  • Have others wait in the car or at home.

  • If the waiting room is full, wait in the hall or your car and call to let us know you have arrived.

If You Have a Mask or Face Covering, Please Bring It
The CDC is now advising that a cloth face covering may help prevent the spread of COVID-19 among asymptomatic individuals. Please bring a mask or a home-made cloth face covering to your visit if you have one.

For your safety, we are

  • Limiting the areas patients occupy during their appointment

  • Maintaining social distancing

  • Monitoring employee health

  • Sanitizing all contact areas before and after patient interaction
